Overview of the Clinical Research Industry

Unit
  • Clinical research: The systematic investigation of drugs, devices, or biologics in human subjects to evaluate safety and efficacy. It is the backbone of medical advancement and regulatory approval.
  • Key stakeholders: Sponsors (pharmaceutical companies, biotech), contract research organizations (CROs), investigative sites, regulators (FDA, EMA), ethics committees, and patients.
  • Global landscape: Clinical trials are conducted worldwide, with emerging markets in Asia and Latin America. Each region has unique regulatory and cultural considerations.
  • GCP compliance: Good Clinical Practice (GCP) is the international ethical and scientific quality standard for designing, conducting, and reporting trials. Adherence is mandatory.
  • Role of the CRA: The CRA acts as the sponsor's representative at investigative sites, ensuring trial conduct follows protocol, GCP, and regulations. CRAs monitor data integrity and subject safety.
  • ICH guidelines: The International Council for Harmonisation provides E6(R2) guidelines that outline CRA responsibilities, including site selection, monitoring visits, and documentation.
  • Career path: CRAs progress from junior to senior levels, then to lead CRA or clinical operations management. Certification (ACRP, SOCRA) enhances credibility.
  • Industry trends: Technology (eSource, risk-based monitoring), decentralized trials, and patient-centric approaches are shaping modern clinical research.
  • Informed consent process: The CRA verifies that the informed consent form (ICF) contains all required elements and is signed and dated by the subject or legally authorized representative before any study procedures. The CRA also ensures that the consent process is properly documented and that updated ICFs are used as approved.
  • Source data verification (SDV): The CRA compares data recorded in the case report form (CRF) against source documents to ensure accuracy, completeness, and consistency. SDV is a critical monitoring activity that detects errors and omissions, thereby protecting subject safety and data integrity. The extent of SDV may be guided by risk-based monitoring approaches.
  • Site selection and initiation: The CRA participates in feasibility assessments, site qualification visits, and site initiation visits (SIV) to evaluate investigator qualifications, site resources, and subject recruitment potential. During SIV, the CRA trains staff on the protocol, GCP requirements, and study procedures to ensure a smooth trial start.
  • Adverse event management: The CRA ensures that all adverse events (AEs) and serious adverse events (SAEs) are reported according to protocol, GCP, and regulatory timelines. The CRA reviews source documentation for complete AE/SAE information, including onset, duration, severity, causality, and outcome, and follows up as needed to ensure subject safety.
  • Regulatory document management: The CRA oversees the collection, review, and maintenance of essential documents such as regulatory approvals, investigator brochures, delegation logs, and training records. Accurate and up-to-date regulatory files, including the investigator site file and trial master file, are crucial for audits and inspections.
  • Data integrity and query resolution: The CRA works with site staff to resolve data queries, discrepancies, and missing information in the electronic data capture system. Timely query resolution is essential for clean data lock and statistical analysis. The CRA also ensures compliance with the data management plan and promotes accurate source documentation.
Regulatory Note

Regulation: FDA 21 CFR Part 312 governs investigational new drug applications, while ICH E6(R2) provides the GCP framework. Sponsors and CRAs must comply with these standards to protect subjects and ensure data reliability.
